Services

Chosen Generations, Inc. offers services to both adults and children who has either the Supports for Community Living Waiver or the MiChelle P. Waiver. Our focus is to meet the individualized needs of each individual who chooses to become a part of our family.

Family Home Provider

Residential level II:Residential Supports provide 24 hour supervision, and assist in activities such as laundry, routine household care, self-care, shopping, money management, socialization, and leisure activities in the provider’s home. No more than three individuals receiving waiver services can live in any residential setting.

Staffed Residence

This service provides 24-hour residential supports in a company owned or leased home called a Staffed Residence. This home can support up to three (3) participants.  Homes can be modified in order to support the person properly. Some supports include, but are not limited to, daily care, medical support, mental health support, meal preparation and development, and community support.

Technology Assisted Residences

Independent living and our team works with you and your team to customize technology for your abilities and needs. Your profile is individualized and tailored to you. There is 24/7 monitoring provided with daily check ins that are convenient and offered on a variety of platforms. Assistance is available withing a reasonable amount of time in the event of an emergency.

Community Access

Community access is a service designed to support an individual to participate in meaningful routines, events, and activities through various community organizations. Community Access is designed to empower a participant in developing natural supports within groups and organizations and the community access specialist fading out after the participant is well established within a group with support. The service stresses training that empowers a participant in acquiring, practicing, utilizing, and improving skills related to connecting with others, independent functioning, self-advocacy, socialization, community participation, personal responsibility, financial responsibility, and other skills related to optimal well-being as defined in the participant’s person-centered service plan.

Respite (SCL/MPW)

Available for a person who does not receive residential services and resides in their own home or their family’s home. This service is a short term support that is needed due to the absence or need for relief of an individual providing care to a participant.

CLS

The purpose of this program is to provide training or assistance to an individual who has the desire to reach specific goals. This can include a variety of activities such as shopping, household care, taking part in community activities, or other daily living skills that the individual needs to become independent. Direct Support Professionals are trained on the needs of the individual in hopes of assisting individuals with disabilities to achieve their own personal goals while in the community. By participating in community wide events and functions individuals with disabilities become more active citizens in the community. CLS services are based on individual needs using a person centered approach. Services occur based on the desired schedule of the individual.
