Chosen Generations, Inc. was founded in 2021 but obtained certification in 2022. Our agency currently services the Louisville, Ky. area with hopes to expanding in the near future.

Mission Statement

Our mission is to advance the dignity of individuals with disabilities by enabling the individual to be as independent as possible. This is accomplished by offering individuals high quality, person-centered services and supports that will facilitate their growth and independence, regardless of the mental, physical, and emotional challenges they may face.

Vision

To make connections and foster relationships with those we serve, their families, our employees, payors, and the communities in which we work to help individuals shape the direction of their own lives in community-based settings.
